1. Designated agent #

2. Filing a notice #

- Your name, address, phone number, and email address.
- A statement that you have a good-faith belief the use is not authorized by the copyright owner, its agent, or the law.
- A statement, under penalty of perjury, that the information is accurate and that you are the owner or authorized to act on the owner's behalf.
- Your physical or electronic signature.
3. Counter-notice and repeat infringers #
If material you posted was removed, you may send a counter-notice with the elements required by 17 U.S.C. § 512(g). We terminate the accounts of repeat infringers in appropriate circumstances.
